Together, we are piloting a groundbreaking approach that empowers seniors with dementia to live safer, longer, and more independent lives at home. This collaboration introduces a new dementia care model inspired by the GUIDE Model, creating a foundation for a significant shift in senior care.

This model empowers families and patients by offering virtual nursing support throughout the dementia care journey. It reduces hospital visits and the need for nursing home stays, while enhancing care quality at home.
