Right at Home Prosper is offering personalized in-home care and two new tools designed to make life easier: the GUIDE Model for dementia support and RightAssist, a smart wearable wrist bracelet that offers monitoring reassurance to loved ones.

The GUIDE Model

What is the GUIDE Model, and who is it designed to help?

Banks: The GUIDE Model (Guiding an Improved Dementia Experience) is a national initiative by Medicare that provides 100% free support for people living with dementia and their caregivers. The model includes care coordination, caregiver education, and respite services, along with connections to community resources when available.

We’re part of the larger GUIDE Model providing respite services. However, the program is larger and multi-tiered, offering services to dementia patients on traditional Medicare.

What are respite services, and why are they important in the GUIDE Model?

Banks: Respite services involve someone coming into the home to offer temporary relief, or respite, for the primary caregiver. They provide the same care—help with toileting, grooming, cooking, light housekeeping, or companionship—to allow the caregiver a break.

Caregiving for someone with dementia can be overwhelming and cause burnout. Through the GUIDE Model, families can get up to 72 hours of respite care a year. It gives caregivers peace of mind, lets them recharge, and ensures their loved one is cared for while they step away.

How can families start the GUIDE Model?

Banks: Call our office at 833-923-2273 to connect with our partner, PocketRN, who will check eligibility and benefits. You need traditional Part A and B Medicare, and it takes about 60 days to start the process, so begin sooner rather than later.

RightAssist Program

What is the RightAssist program, and how does it help families monitor seniors at home?

Banks: RightAssist is a simple, waterproof light bracelet worn 24/7 with a nearly year-long battery life. It monitors activity 24 hours of sleep patterns, gait speed, bed exits, repositioning, and exercise minutes. In the case of a fall, an alert is sent immediately to family members. By reviewing these insights regularly, families and caregivers can spot potential health concerns early and take preventive action. We review the reports weekly, and if we see anything high-risk, we reach out to the family. It gives families an extra set of eyes and helps seniors stay safe at home.

How does RightAssist help seniors age safely at home?

Banks: The key to aging well is to be proactive, not reactive. RightAssist is a pre-care tool that tracks key health indicators, helping families and caregivers monitor seniors’ well-being and detect changes over time. By closely monitoring a senior’s health, you can help prevent catastrophic events that could drastically change their lifestyle.
